To sign-up for the DVC SPRING CLINIC (3 requirements)
1. Please fill out and submit the sign-up information at the bottom of this page to sign-up for the DVC SPRING CLINIC. (Submitting your information below is confirmation that you are signed-up for the clinic.) Please make sure you click on the "submit" button at the bottom of the page.

2. CLICK HERE for our paypal link to pay for the DVC SPRING CLINIC. Must pay $55 fee on-line. AS OF APRIL 21, 2014, NO T-SHIRT $30 FOR REMAINING DATES.

3. CLICK HERE to Print the assumption of risk form from this website. Sign and bring it to your first clinic session.
