COMPLETE #1 ,2, 3, & 4 BEFORE ATTENDING OPEN GYM OR TRYOUTS

    1. You must bring an assumption of risk form to the first open gym.  CLICK HERE for assumption of risk form for you to print and sign.

    2.  You must pay on-line the $50 open gym fee(includes all open gym dates & tryouts.)
         **If a player is unable to attend any open gyms in October, she must pay the one day $50 tryout fee.)

    Open gym/Tryout fee must be paid on-line.

    3.  You must register with SRVA at srva.org (expected to be on-line Sept. 19)(then digitally sign and print the membership form (click on yellow bar on upper left to print) and bring the form to open gym with you.

    4. You must submit the PLAYER information at the bottom of this page. (please make sure you click the submit button at the bottom of this page). 
    *You must use the age calculator to determine the age team for which you will tryout.  It is not the age you are currently.
