To sign-up for the HIGH SCHOOL CLINIC FOR RISING 9TH-12TH GRADE GIRLS (3 requirements)

2. CLICK HERE for our paypal link to pay for the DVC HIGH SCHOOL CLINIC. Must pay $20 fee on-line.

3. CLICK HERE to Print the assumption of risk form from this website. Sign and bring it to your first clinic session.
